Summary:
Essential Functions:
Home Evaluation and Modification:
Work with consumers to evaluate needs for home modifications, while assessing the home’s feasibility for the needed modifications.
Ensure that residential renovations adapt existing homes to meet the needs of persons with disabilities, providing maximum independence.
Complete a home modification scope of work.
Assemble home modification proposals for funders.
Effectively read and understand design plans.
Contractor Management:
Identify appropriate contractors for the proposed scope of work.
Work with contractors to ensure modifications are done correctly and on time.
Participate in the development and implementation of annual home modification trainings for contractors.
Compliance and Reporting:
Enforce all program requirements of the PA Office of Long-Term Living.
Adhere to all federal, state, and local requirements such as ACHC, HIPAA, OSHA, etc.
Complete monthly home modification reporting.
Inventory and Product Knowledge:
Maintain accurate data on accessibility adaptation equipment inventory.
Advocacy and Representation:
Advocate for consumer control policy in the home modification process.
Represent Liberty Wheels, LLC at designated functions, community events, and advocacy and community education activities as directed.
Perform other work-related duties as assigned.
